Important Things to Know
Appointments: All office visits are by appointment only and you can have an appointment on the same day you call. To schedule an appointment call one of our receptionists at (330) 759-7038. Please make every effort to keep your appointment, as “No Shows” are subject to a $20-$50 penalty fee. If you are a new patient, obtain your previous medical records, including recent laboratory tests, prior to your appointment.
If you are unable to keep your appointment, please call us at least 24 hours in advance, as a courtesy to other patients who may need urgent care.
We know that your time is important to you, so we strive to stay on schedule with your appointment. Sometimes, however, unavoidable delays occur. We will keep you informed if a delay arises. We appreciate your understanding in situations like these.
Telephone Calls: All messages are received as soon as possible, and every effort is made to return calls during regular working hours. There are times when we cannot answer all calls within the same day they are received.
Please tell us if your call is urgent, or must be returned the same day. Please confirm your contact telephone number with our receptionists.
Prescriptions, Refills, and Prior-Authorizations: On your first visit, please bring a list of all the medications and supplements you are currently taking. Advise us if you have any allergic reactions to any medications.
Medication refills are done during office visits. If you call the office or have your pharmacy call for your medication refills, allow 1-2 days processing. You must also pay $8.00 via cash or credit card for all non office visit refills. No refills will be made after office hours because your medical records may not be available.
We do not prescribe narcotics.
Referrals and Pre-Certifications: We work closely with other physicians in the community and will sometimes need to refer you to a specialist. Those patients with managed care insurance, who require routine referrals, please provide us with 5 business days notice.
Depending on your insurance, some radiological exams will require Pre-Certification. Your exam cannot be scheduled without your Pre-Certification. One of our nurses must call your insurance company to obtain approval. It takes extra time, so please be patient with this process and allow us ample lead time.
Billing and Insurance: Please bring your insurance card with you and your co-pay amount at the time of each visit. Our contract with the insurance companies requires your co-payment at the time of each visit.
Confidentiality: All your medical records are confidential. We can release your records only if given written authorization. By law we may not discuss your medical care with anyone, including family members, without your permission.
On Call Schedule: A Physician is "on call" whenever the office is closed. Please reserve the "on call" service for emergencies only or medical situations that cannot wait until the office reopens.
The "on call" doctor can be reached through the office answering service by calling (330) 742-7608. Please have your pharmacy number ready in case phone-in medication is required. If you do not hear from the "on call" physician within 30 to 45 minutes, please call the answering service again to make sure there was no technical problem or a wrong phone number.
Routine prescription refills will not be made by the on call physician.
Patients with life threatening symptoms should call 911, or go directly to the nearest Emergency Room.
You may be charged for non-emergency or non-urgent phone calls to the physician after the office is closed.
